Take some time to get to know some of the wonderful staff members here at the Forest City Nursing & Rehab Center. These are only a few of the individuals who serve our residents on a daily basis. There is a long list of over 100 team members who contribute to meeting our organization's goals and ensure continuous ongoing success. Drew Sparks, LNHA, facility administrator and owner has been involved with our facility since 1984. Through the years, it's been Drew's philosophy that the resident's needs always come first. His main priority is that residents and families are pleased with the exceptional care and services delivered by Forest City Nursing & Rehab Center and Forest City Personal Care. Drew resides in Clifford Township with his wife and two sons. He enjoys spending time with his family, being outdoors, golfing and riding his motorcycle.
Assistant Administrator of Resident Services
Lisa Demchak, RN, NHA has served the residents of Forest City Nursing & Rehab Center for the past 30 years. She started her career as a nursing assistant, obtained her nursing degree from Penn State University, and is currently employed as the Assistant Administrator of Resident Services. Lisa does not manage behind her desk but takes an active hands-on approach to ensure the highest quality of care is delivered to our residents. In her spare time Lisa enjoys traveling and time with family, especially her two children.
Director of Nursing
Susan Loomis, Director of Nursing has more than 15 years as a Director of Nursing as well as experience in multiple other long term care positions. She is proud to be able to say she began her long term care career here in 1984 as a nursing assistant. Her knowledge and expertise make her one of the most valuable and respected members of our team. She is the first one to lend a helping hand or offer a solution to a problem. Sue and her family reside at Lake Wallenpaupack. When not working, she enjoys teaching Sunday School, taking beach vacations, and most of all spending time with her daughters and grandchildren.
Director of Social Services
Mary Ann Rogan
Mary Ann is a graduate of Marywood University with both a Bachelors and Masters Degree in Social Work. She began her career working in Mental Health before taking a position in the hospital setting. She transitioned to the Forest City Nursing & Rehab Center in 1993 as the director of Social Services. Throughout her experiences at the facility, she has had the pleasure of meeting and assisting many residents and families as they continue their journey through life. She is married with two children and enjoys spending time with her family, taking long walks, as well as cooking and baking.
Jennifer started her career at Forest City Nursing & Rehab Center in 2014 as a Nursing Assistant. While employed as a Nursing Assistant, she recognized that the resident’s medical needs were met by the nursing staff, but their social and emotional needs were the responsibility of the Activities Department. In 2015 she became our certified Activity Director with a mission to bring meaningful quality activities to our resident population.
Prior to Jennifer’s career in long term care, she spent almost a decade in South Florida as a licensed SCUBA instructor; she taught and certified over 1000 divers. She moved back to Pennsylvania in 2005 and worked as a resident aide in a Personal Care home. It was during this time when Jennifer recognized that people, no matter what age they are, still have the desire to learn new things and have meaningful socialization with others. Jennifer resides in Clifford with her husband Erich and her dog Rider. In her spare time she enjoys snowmobiling and hiking.
Director of Maintenance
Robert started his career at Forest City Nursing & Rehab Center as our Maintenance Director in 2016. Rob has earned two associate degrees in EET and telecommunications and has spent over 33 years working in industrial maintenance. Rob has resides in Browndale with his wife, Jackie, and their very sweet and loving Rottweiler, Emma. Rob has one daughter, Kelsey, who is currently making her career in the business world. In his spare time, rob enjoys hunting, fishing and playing golf.
Assistant Administrator of Operations, Personal Care Administator
Mia Crotti has been with Forest City Nursing & Rehab Center and Forest City Personal Care since 2010. Mia received her B.S. in Health Science with a concentration in Health & Wellness Management from the University of the Sciences in Philadelphia. She also received her Master’s in Health Administration from the University of Scranton. Mia holds her Nursing Home Administrators license as well as her Personal Care Home Administrators license and currently functions as the Assistant Administrator of Operations. In her leisure time she enjoys physical fitness activities such as crossfit, soccer, and training for fitness competitions. The residents enjoy visits from her furry, four legged, K9 best friend, also known as Colbert.
Director of Environmental Services
Billie Jean Wilbur
Billie, our Director of Environmental Services, works with Healthcare Services Group, Inc (HCSG) in our facility and she is a graduate from Susquehanna Community School. Billie’s states that she greatly enjoys her employment at FCN&RC; her positive attitude has helped her excel as a team leader with HCSG since 2012. Outside of work, she enjoys spending time with her family and some of her most favorite outings include riding quads, camping, and playing games.
Director of Rehabilitation
Brandi Schiavone DOR, has been our Director of Rehabilitation since 2011. Prior to her management role, she has been a staff occupational therapist here since 2005. She is a graduate of Misericordia with a Master of Science in Occupational Therapy and has spent the past 15 years working in all therapy settings with a true niche for skilled nursing/long term care. While Brandi considers the Forest City Nursing and Rehab Center her home away from home, she enjoys spending time in her actual home at Crystal Lake with her husband John and three children: Darien, Alyvia and Jake.
